He talks about in his book, he was in a car accident that left scars on his head that made his hair grow in patches. He later went bald. But that is why he originally started wearing hats.
Steven has said that he cant grow hair because of a childhood car accident and always wears a colorful scarf over his head because of it. I think David sacrificed the ability to make hair piece jokes with Sil's character throughout the series because he knew he had the perfect Sil in Van Zandt.
